Uw gegevens: Bij een bezoek aan onze praktijk verzoeken wij u altijd uw zorgpas mee te nemen en een geldig legitimatie bewijs.
Wijzigingen: Wilt u wijzigingen in uw gezondheid-,naam-,adres-, telefoon- of verzekerings- gegevens zo spoedig mogelijk aan ons doorgeven. U bent er zelf verantwoordelijk voor dat wij de juiste gegevens van u hebben.
Afspraak maken: Voor het maken van afspraken is de praktijk op werkdagen telefonisch bereikbaar. Wanneer u een afspraak heeft, verzoeken wij u op tijd aanwezig te zijn. Wij sturen u 1 week en 1 dag voor de afspraak een herinnering per e-mail.
Afspraak afzeggen: AUB zeg de afspraak uiterlijk 24 uur van te voren (op werkdagen) telefonisch af! Doet u dit later, dan moeten we u helaas kosten in rekening brengen. Deze zijn afhankelijk van de behandeling die ingepland stond.
Personal details: When visiting our dental practice, we kindly ask you to bring your health insurance card and a form of identification with you.
Changes: We would like to ask you to inform us as soon as possible if anything regarding your name, address, insurance policy or any health changes. You are the one who is responsible that we have your correct personal information.
Booking an appointment: We are available by phone during workdays. If you have an appointment, we sincerely ask you to be punctual. We will send you a reminder of your appointment 1 day and 1 week in advance.
Cancellation policy: We ask you to cancel your appointment 24 hours in advance (on workdays) by calling us! If you do not manage to do that, we unfortunately have to charge you. The costs depend on the treatment that was scheduled.
